How to Become a Client

We are currently accepting clients at the South Georgian Bay Community Health Centre in both our Wasaga Beach and Elmvale locations. We have made our intake process as simple as possible.


If you are wanting to become a client, please read through the following steps:

  1. Drop by (14 Ramblewood Drive Unit 202 Wasaga Beach) or call (705 422 1888) and let reception know that you would like to become a client. 
  2. They will ask for a few contact details and book you an intake appointment.
  3. Be sure to let them know if you have another health care provider (family doctor or nurse practitioner) and they will help you de-register before your intake appointment.
  4. Come to your intake appointment. At this point, you will be asked to fill out several intake forms and you will meet with your nurse practitioner or physician (dependent on availability and the complexity of your health needs). Please let us know if you require any assistance filling out the forms. 
  5. Upon completion of your intake appointment, you will have access to all of our services (social workers, mental health and addictions counsellor, health promoter, dietitian, diabetes educators, and chronic conditions nurse).

*Note that we are not a walk in clinic and we run by appointment only. You must be a registered client with the SGBCHC to have an appointment with a nurse practitioner, registered nurse, or physician.

Target Population

While we welcome everyone as clients, and our team's work in the community will be of benefit to the entire population, first priority for primary health care services (i.e. the kinds of assistance you would get through a family doctor) will be those individuals facing special barriers accessing health care, namely:

  • Youth between the ages of 12 and 24
  • Individuals or families experiencing poverty, housing and/or food security challenges
  • Individuals or families experiencing mental health and/or addiction issues
  • Individuals with any disability that presents a barrier to wellness or service access, and
  • Isolated seniors
