Ontario Health Teams are being introduced to provide a new way of organizing and delivering care that is more connected to patients in their local communities. Under Ontario Health Teams, health care providers (including hospitals, community health centres, doctors and home and community care providers) work as one coordinated team.

The South Georgian Bay Ontario Health Team (SGB OHT) is a partnership network of health and social service community organizations in South Georgian Bay that work together to improve health and social care for the people of South Georgian Bay. We strive to be the healthiest community in Canada.
