Who We Are

The South Georgian Bay Community Health Centre (SGBCHC) is part of a growing network of Ontario’s Community Health Centres (CHCs). Each CHC is as unique as the community it serves. With over 107 community-governed primary health care organizations across the province, the CHC Model of Care is proven to build healthy communities.

At the SGBCHC, we focus on the health and well-being of individuals in our community, particularly those who are vulnerable. We do this by providing everyone in our community with access to health care services, free community programs and community development initiatives.

We serve the communities of Wasaga Beach, Clearview, Collingwood, Elmvale, and the Blue Mountains. Our main clinic is located in Wasaga Beach and a second point of access is available every Wednesday in Elmvale.

Our multi-disciplinary health care team is made up of physicians, nurse practitioners, nurses, social workers, dietitian, physiotherapist, health promoters and other health care professionals and support staff, which provide our clients with access to the right care, at the right time, at the right place.

We recognize that health care is more than treating illness. Being healthy also means having the opportunity to work, socialize, eat healthy, learn and contribute to one’s community. Working together with our clients and our community is at the heart of our model of care, because in Community Health Centres: Every One Matters.
