Self Management Programs

All of our programs are free of charge and open to everyone in the community. If you have issues accessing these programs such as transportation, physical or language barriers, please let our program staff know and we will work with you to eliminate this barrier. Brief Action Planning

The Brief Action Planning workshop provides health care professionals with three practical, highly structured, patient-centered, evidence based self-management support tools grounded in the principles and practice of motivational interviewing.

Choices & Changes

Clinicians frequently express frustration about their capacity to influence patient behavior, especially when they see the negative impact of that behavior on their patients’ health outcomes. Choices and Changes is a state of the art workshop supported by extensive research that provides clinicians with specific, brief and efficient communication strategies that can be utilized as part of a typical office visit.

Living a Healthy Life with Chronic Conditions

This free 6-week program is for anyone living with any chronic condition. This could include diabetes, heart disease, arthritis, lung disease, as well as many other chronic health issues. Each week individuals learn skills that can help them better manage their symptoms as well as their daily lives. Living a Healthy Life with Chronic Pain

This free 6-week online program is for anyone living with chronic pain. This could include but is not limited to lower back pain, pain from motor vehicle accidents, arthritis, migraines, fibromyalgia, and other pain-related conditions. Each week individuals learn skills that can help them better manage their chronic pain symptoms, as well as challenges associated with chronic pain and improve their quality of life. Family members and/or caregivers are also welcome.

Motivational Interviewing

The Founders of Motivational Interviewing recommend regular practice and feedback to be a crucial component to support skill development. The quarterly practice sessions are utilized to emphasize a specific area of MI that is of interest to the group and offers an opportunity for practice and feedback.

Powerful Tools for Caregivers

Powerful Tools for Caregivers is an evidence-based workshop that offers the skills and tools for taking care of yourself while also caring for someone else. By taking care of your own health and well-being, you will be a better caregiver. This workshop runs once a week for 6 weeks and is led by trained facilitators.

Social Support Group

This 6-week program is an online and phone-in group for social connectedness. The workshops are developed for graduates of the Living a Healthy Life programs, but anyone living with a chronic health condition or chronic pain is welcome to join.
The leaders will integrate structured activities into the sessions, including brief check-ins, relaxation activities, breathing exercises, action planning, decision-making and problem-solving. Additional activities will be based on participant interests.
