Self Management Programs

All of our programs are free of charge and open to everyone in the community. If you have issues accessing these programs such as transportation, physical or language barriers, please let our program staff know and we will work with you to eliminate this barrier. *Virtual Living a Healthy Life with Chronic Conditions

This free 6-week program is for anyone living with any chronic condition. This could include diabetes, heart disease, arthritis, lung disease, as well as many other chronic health issues. Each week individuals learn skills that can help them better manage their symptoms as well as their daily lives. Family members and/or caregivers are also welcome. *Anyone can register for this workshop. To register please contact dsm@sgbchc.ca or call 705-422-1888.

This could include but is not limited to lower back pain, pain from motor vehicle accidents, arthritis, migraines, fibromyalgia, and other pain related conditions. Each week individuals learn skills that can help them better manage their chronic pain symptoms, as well as challenges associated with chronic pain and improve their quality of life. Brief Action Planning

The Brief Action Planning workshop provides health care professionals with three practical, highly structured, patient-centered, evidence based self-management support tools grounded in the principles and practice of motivational interviewing.

Choices & Changes

Clinicians frequently express frustration about their capacity to influence patient behavior, especially when they see the negative impact of that behavior on their patients’ health outcomes. Choices and Changes is a state of the art workshop supported by extensive research that provides clinicians with specific, brief and efficient communication strategies that can be utilized as part of a typical office visit.

Motivational Interviewing

The Founders of Motivational Interviewing recommend regular practice and feedback to be a crucial component to support skill development. The quarterly practice sessions are utilized to emphasize a specific area of MI that is of interest to the group and offers an opportunity for practice and feedback.
