Self Management Programs

All of our programs are free of charge and open to everyone in the community. If you have issues accessing these programs such as transportation, physical or language barriers, please let our program staff know and we will work with you to eliminate this barrier. Brief Action Planning

The Brief Action Planning workshop provides health care professionals with three practical, highly structured, patient-centered, evidence based self-management support tools grounded in the principles and practice of motivational interviewing.

Choices & Changes

Clinicians frequently express frustration about their capacity to influence patient behavior, especially when they see the negative impact of that behavior on their patients’ health outcomes. Choices and Changes is a state of the art workshop supported by extensive research that provides clinicians with specific, brief and efficient communication strategies that can be utilized as part of a typical office visit.

Living a Healthy Life with Chronic Conditions

This 6-week program is for adults of all ages who have Diabetes, Heart Disease, Arthritis, Chronic Lung Disease, Asthma, Fibromyalgia, and other long-term illnesses. This peer-led group meets for 2.5 hours every week over 6 weeks. To register please contact dsm@sgbchc.ca or call 705-422-1888.

Living a Healthy Life with Chronic Pain

This 6-week program is for adults of all ages who face the challenges of conditions such as Arthritis, Migraines, Fibromyalgia, and Repetitive Strain Injury, Phantom Limb Pain, Back or Neck Pain, and the list goes on. Motivational Interviewing

The Founders of Motivational Interviewing recommend regular practice and feedback to be a crucial component to support skill development. The quarterly practice sessions are utilized to emphasize a specific area of MI that is of interest to the group and offers an opportunity for practice and feedback.
